Voluntary Critical Incidents Benefit

The Critical Incident Cover is a voluntary Group Life Scheme open to all ISASA Schools. Each school can choose to participate in the Scheme. Once a school participates in the Scheme, all eligible employees must join the scheme.

1. The employer must complete the GAP Onboarding Form to begin the process of launching the scheme.

2. Complete Sections 3 and 12 of the form and provide the following documents, which is required for customer verification purposes:

   • Client Registration Certificate
     
     
   • Mandated Official Documentation:
     
     
     1. Evidence of Authority:
        
        
        • A general board resolution signed by the Company Secretary, Chairman, or a Director, authorising appointed individuals to act on behalf of the customer in relation to Old Mutual.
          
        • A delegation of authority or scheme of delegated authority.

     2. Proof of Address
        
        
     3. ID or Passport Number

On receipt of the above, Old Mutual will continue and get the insurer to launch the scheme.
